Piles Treatment
Piles, also called haemorrhoids, are swollen blood vessels in and around the anal canal. Overview
Haemorrhoids are cushions of blood vessels that sit inside the anal canal and help with continence. They become a problem when they enlarge, bleed, or slip down out of the anal opening. Internal piles usually present with painless bleeding, while external piles are more often felt as a tender swelling.
Piles are commonly graded from first to fourth degree depending on how far they prolapse and whether they return on their own. This grading matters, because the treatment appropriate for early piles is different from what is needed when the piles remain outside the anal opening.
Many patients delay consultation out of embarrassment, and by the time they come the bleeding has been going on for months. Earlier assessment usually means simpler treatment and a clearer explanation of what is happening.
Common symptoms
- Bright red bleeding during or after passing stool, often painless
- A lump or swelling felt at the anal opening
- Itching, moistness or irritation around the anus
- A feeling that the bowel has not emptied completely
- Discomfort or aching after sitting for long periods
- Mucus discharge staining undergarments
Common causes
- Chronic constipation and straining while passing stool
- Prolonged sitting, including long hours of driving or desk work
- A diet low in fibre and inadequate water intake
- Pregnancy and the period following childbirth
- Repeated heavy lifting
- Long-standing diarrhoea or frequent laxative use
How it is assessed
- A detailed history of bleeding, pain, swelling and bowel habit
- Local examination of the anal region, carried out privately
- Proctoscopic examination where it is required to see the anal canal
- Assessment for other causes of bleeding, so that piles are not assumed without examination
- Referral for further investigation if the pattern of bleeding suggests something beyond piles
Treatment approach
- Medical management with prescribed medication for pain, bleeding and inflammation in early or acute presentations
- Correction of constipation through diet, fluids and stool consistency, which is central to preventing recurrence
- Procedural options appropriate to the grade of piles, discussed with you along with what recovery involves

- Follow-up review to confirm healing and to reinforce the habits that keep symptoms from returning
When you should consult a doctor
- Bleeding that continues for more than a few days, or bleeding that is heavy
- A swelling that has become painful, hard or discoloured

- Any change in bowel habit, unexplained weight loss or anaemia alongside bleeding
- Symptoms in pregnancy, where treatment needs to be chosen carefully
